Scope

DHHS’s Latino Health Initiative (LHI) seeks experienced and qualified entities to improve the overall health and wellbeing of Latino and other ethnic and/or racial minority populations residing in Montgomery County by delivering comprehensive, culturally and linguistically competent health promotion and empowerment interventions to these populations through the Latino Youth Wellness Program (LYWP). The goal of this program is to increase overall wellness and to prevent health conditions that disproportionately affect Latino youth and other minority youth by enhancing protective factors. The LYWP implements activities via two components: promotion of healthy behaviors and support, and leadership development, as described further below. The County intends to award up to 3 contracts from this RFP based on these two components:


A. Promotion of Healthy Behaviors and Support- through the provision of services and interventions to low-income families who have high-risk youth ages 11 to 15, such as those with academic and behavioral problems, recently arrived immigrants, ESOL students, and those having-high risk factors associated with one or more negative outcomes regarding the following areas: behavioral health, reproductive health, substance abuse, nutrition, physical activity, and parent-child relationship.

B. Leadership Development- through interventions focusing on youth ages 16 to 21.The aim of the leadership component is to equip participants with the necessary guidance knowledge, skills, and abilities on vocational orientation that promote social integration and civic engagement by actively participating in developing solutions, activities and processes that address the root causes of social problems that impact the Latino youth’s health and wellness in a systematic way to improve overall quality of life and wellbeing of their community.
