Scope

The Montgomery County Department of Health and Human Services (DHHS) requires the following services provided by licensed Certified Nursing Assistants to eligible Montgomery County (County) residents: In Home Aide Services (IHAS).

The County, through DHHS’s Aging and Disabilities Services (A&D), wishes to provide Home Health Aid Services. The target population for services provided under this Request for Proposals (RFP) are eligible frail seniors, and adults with physical and mental disabilities. This includes individuals who are chronically and acutely ill; individuals living with functional disabilities; individual’s living with physical, intellectual, and/or emotional disabilities; elderly clients who are frail’ and adults with special needs, aging or disabled adults who are County residents with physical or intellectual limitations. The IHAS intends to maximize the population’s levels of self-sufficiency and ensure that they receive protection and other services to prevent abuse, neglect, self-neglect, exploitation, or inappropriate in situations.

The DHHS Aging and Disabilities Service Home Care Unit, will determine clients’ eligibility for the services to be provided under this RFP. The DHHS will refer Clients to selected contractors for IHAS. For each client referred, a DHHS social worker will complete an initial assessment of need and eligibility for services. The DHHS staff will discuss the care plan and other client information with the Contractor at the time of referral.
