Step 1: Select a Location

The Back-to-School (BTS) Appointment Booking System will allow you to choose the location, select an appointment date/time, submit essential information necessary to schedule a BTS vaccine appointment, and generate an appointment confirmation email notification confirming your appointment. As you move through each step to make your appointment, a status bar toward the top of the web page will indicate your progress.

Please register each family member/patient name separately.

The following vaccines are FREE: The immunization team will verify the vaccine/s your child needs on the day of your appointment.

    The Tdap, MCV4 and Varicella are required for school.
  1. Tdap - students 7th through 12th grade if they have not received a Tdap vaccine between age 11-12 years of age. Valid if received at 10 years of age.
  2. MCV4 – students 7th through 12th grade if they have not received a MCV4 vaccine between age 11-12 years of age, 2nd dose at age 16-18 years.
  3. Varicella (chickenpox) – first dose on or after their first birthday, then a 2nd dose 4-6years of age. Note: two (2) doses of varicella vaccine are required for students entering Kindergarten through 10th grade and for previously unvaccinated student 13 years and older. Medical diagnosis of varicella disease is acceptable in lieu of vaccination.
    4. The Men B, HPV, Flu and COVID-19 are recommended vaccines.
  4. Men B – At 16-18 years of age, followed by a 2nd dose in a month.
  5. HPV – starts at 9 years of age.
  6. Flu (injectable) – starts at 6 months of age (begins in August 2024).
  7. COVID-19 – the new updated 2023-2034 formula – starts at 6 months of age.
